Building Inspection Readiness Into Everyday Clinical Operations

25 October 2026

Quality and clinical staff reviewing an organised trial master file together.

Inspection readiness that starts when the notification arrives is reconstruction. Inspectors can tell. The trial master file may be complete and still tell a story of decisions that were late, unsigned, or inconsistent with the monitoring record.

Readiness is a by-product of ordinary operations. Monitoring reports exist and were reviewed. Deviations have a classification and an outcome. CAPA has an owner and evidence of effectiveness, not only a plan. Vendor oversight is documented at the moments it happened. Someone can explain, from the file, what the sponsor knew and when.

Put it in the week, not the war room

That standard belongs in sponsor oversight and in day-to-day monitoring, including follow-up that actually closes. It also belongs in the way issues are managed under clinical trial management. Quality is not a separate season at the end of the study.

Teams who have hosted audits and inspections learn the same lesson: the file is persuasive when the operations were. A positive inspection outcome is earned in the months when nobody was watching.
